2015 Florida Annual Conference Event Child Care

The Florida Annual Conference will offer child care for Elementary and Pre-school age children, Wednesday, June 10th through Saturday, June 13th at First UMC Port Orange.

Pre-Registration for Child Care Is Required
Pre-Registration Deadline:  Friday, May 15, 2015
In order to receive this service, you MUST pre-register all children requiring child care. If you have not pre-registered, we will be unable to provide child care. Pre-registration ensures that there is an adequate number of child care workers on duty to meet the child/worker ratio (required by law) and also prevents too many workers on duty, which hurts this program financially. If your need for child care changes from your pre-registration request, please notify Samantha Noonan so they might offer childcare for others needing this service.

$85 per child for all four days
Additional Children $70 per child
Family Maximum  $225

Notarized Medical Care Authorization must accompany pre-registration for each child registered.

COST: An $85 non-refundable registration fee covers your child care cost for the weekday annual conference business sessions, whether you need child care for one day or for all 4 days. Add $70 for each additional child. (For example, the cost for child care for two children for the three days would be $155; three children would be $225.) Family Max $225.

WHAT TO BRING:

• Children under age 5 will need to have a copy of their health and shot record with them when they arrive (to be in compliance with our local licensing agency).
• Bring any necessary clothes or diapers. It is essential that you label your child’s items such as clothing, bottles, bags, pacifiers, etc.
• Child care through the lunch break will be offered. Lunch will not be provided. Please bring a bag lunch for your child.
