Minnesota Life Insurance Company offers participants the opportunity to purchase Life and Accidental Death and Dismemberment (AD&D) insurance during Open Enrollment. Coverage is available for active clergy and lay employees (working at least 30 hours per week) and their immediate family members.  The guaranteed coverage amount can be purchased without Evidence of Insurability (EOI) only during the initial enrollment period. If you decline coverage when you are first eligible for voluntary life insurance you will have another opportunity to enroll during our annual enrollment period. However, you will then need to submit an Evidence of Insurability form and the insurance company will make a determination on your eligibility for coverage.
