Listed below are links to two Apportionment Giving Report forms.
The first form is an interactive .pdf form that can be filled in on your computer and then printed. This form will also add your entries. You will enter the check total in the amount enclosed field.
Print a copy to submit with your check as well as a file copy.
The second form is simply a blank report form in pdf format that you can print out and then fill in either by hand or by typewriter.
Submission instructions and address are included on each form.
